You have been registered on the Kaiser Permanente Individuals and Families site. To apply online for coverage, just follow these seven easy steps.

Note: Before applying, please disable the pop-up blocker option in your Web browser, or create an exception for this site. When you are filling out the application, please use the buttons located at the bottom of each page to guide you. Do not use your browser's "Back" or "Forward" buttons—this could result in incorrect information and delay your application.

Step 1:  Click on the URL below and use the provided user ID and password to sign in to our secure Web site.

Step 2:  Before continuing with the application, please change your password by clicking on the green "Change password" button. For more information about our commitment to your privacy, please read our Privacy Practices.

Step 3:  After changing your password, read the Applicant Information page and click the "I Agree" button.

Step 4:  Select "New" as the type of account and then click continue.

Step 5:  Select your plan type and click continue.

Step 6:  Choose the person(s) you want to cover under the Kaiser Permanente Individual and Family Plan and click continue.

Step 7:  Complete a medical questionnaire for yourself and for each family member applying for coverage. You can complete the questionnaire(s) online, or you may print it out and mail it in.
